使用人工鼻联合持续气道湿化技术湿化液量探讨陈立萍宗永忠李青荷林娟孙兰(东南大学医学院附属盐城医院江苏盐城)【摘要】目的探讨不同气道湿化量对使用人工鼻联合持续气道湿化技术机械通气患者的影响。方法将我院重症医学科80例使用人工鼻联合持续气道湿化技术的机械通气患者随机分为两组,其中观察组40例通过评估病人痰液情况随时调整湿化量,痰液粘稠度I度以2・4ml/h、II度以4・6ml/h、III度以6・8ml/h的速率泵入;对照组40例直接以2ml/h的速率泵入湿化,对两组患者的湿化效果、气道黏膜出血、痰痂形成情况、刺激性咳嗽、呼吸机相关性肺炎(VAP)的发生率、导管更换频次、平均每例每天吸痰次数进行对比。结果观察纟fl湿化效果好,气道粘膜出血、痰痂形成惜况、导管更换频次、平均每例每天吸痰次数少,刺激性咳嗽、呼吸机相关性肺炎的发生率低,差异有统计学意义(PV0.05)。结论机械通气患者采用人工鼻联合持续气道湿化技术,通过评估痰液粘稠度及吋调整湿化量,保证了人工气道的湿化效果,有利于预防相关并发症的发生,有效地降低了医院感染发生率,同时也减轻了患者痛苦,缩短了住院吋间,减少了护理工作量,人人提高了医疗护理质量,值得临床应用推广。【关键词】机械通气;人工鼻;联合;气道湿化;湿化量;临床研究【中图分类号】R473.5【文献标识码】B[文章编号】Studyofthehumidificationfluidvolumebyartificialnosecombinedcontinuousairwayhumidification*CHENLi-ping,ZONGYong-zhong,LIQing-he,LINJuan,SUNLan.(TheThirdPeople'sHospitalofYancheng,Yancheng,Jiangsu224001,China)[Abstract]ObjectiveToexploredifferentairwayhumidificationquantityontheuseofartificialcombinednasalcontinuousairwayhumidificationtechnologyinpatientswithmechanicalventilationeffect.Methods80casesoftheuseofartificialcombinednasalcontinuousairwayhumidificationtechniquesofmechanicalventilationwererandomlydividedintotwogroups.Accordingtothepatient^sputumviscosityadjustmenthumidificationquantity,Thehumidificationfluidof2-4ml/h,4-6ml/hand6-8ml/hweregivenintheobservationgroup40cases.Thehumidificationfluidof2ml/hratewasgivendirectlyincontrolgroup・comparativeanalysisoftwogroupsofhumidification,airwaymucosalbleeding,thephlegmscabformation,irritatingcough,venti1ator-associatedpneumonia(VAP),theincidenceofcatheterreplacementfrequency,averageeverydaythenumberofcasesofsputumsuction.Resultstheobservationgrouphumidificationeffectisgood,theairwaymucosalbleeding,thephlegmscabformation,catheterreplacementfrequencyandreducethenumberofsputumsuction,irritatingcough,ventilator-associatedpneumoniaincidenceislow,thedifferencewasstatisticallysignificant(P<0.05).Conclusionthroughtheevaluationofsputumviscosityandtimelyadjustmentofhumidificationquantity,guaranteestheartificialairwayhumidificationeffect,isconducivetothepreventionofassociatedcomplications,effectivelyreducingtheincidencerateofhospitalinfection,reducethesufferingofpatients,reducethenursingworkload,improvethequalityofmedicalcare,itisworthyofclinicalapplication.[Keywords]mechanicalventilation;artificialnose;joint;airwayhumidification;humidificationquantity;clinicalresearch机械通气患者使用人工鼻联合持续气道湿化可以保持人工气道的病人呼吸道内恒定的温度和湿度,降低痰液粘稠度,避免痰痂性形成,有利于保持呼吸道通畅,减少吸痰次数及时间,从而使气道粘膜损伤减少,细菌生长繁殖机会减少,降低VAP的发生率。临床上采用人工鼻联合持续气道湿化技术湿化液矍很难掌握,滴速太慢,达不到湿化效果;滴速太快,对患者刺激性人,吸痰及咳嗽次数增加,影响通气效果,患者也难耐受n我科自2011年06月至2011年12月对按病情需要机械通气并且可以使用人工鼻联合持续气道湿化技术的患者进行了湿化液量的研究,现总结报道如下:1...
