影响Sysmex-18001血液分析仪血•小板计数因索复检原则的探讨［摘要］目的:探讨并建立合适的影响Sysmex-lSOOi仪器血小板计数的复检原则。方法:利用Svsmex-1800i血细胞分析仪对450例血液样木进行分析，分为红细胞不同的MCV组、血小板内在因素组、采血因索组。并对备组血小板进行显微镜手工计数及相应的血涂片复查。结果：①MCV280fl和70fl^MCV<80fl时，仪器与手工法结果间差异无统计学意义(P>0.05);60flWMCV<70fl时，仪器法与手工法间差异有统计学意义(P<0.05);MCV<60fl,仪器法与手工法间差异有统计学意义(P〈0.05)。②血小板直方图异常及大血小板，仪器法与手工法间差异有统计学意义(P〈0.05);血小板聚集时仪器所测数值与血片所见血小板数量差异显著。③不同采血方式血细胞分析检测结果的影响如在可接受的范围Z内，利用EDTA-R2真空负压管采集静脉血具有结果稳定、样木量多、便于复杳等诸多优点，在优先静脉采血的同时可根据临床实际情况选择样木类型及采血容器。当出现血小板减少的数据,应重采静脉血，并进行手工血小板计数。结论:红细胞MCVW70fl或者血液标本不合格及血小板结果和临床不相符时，有必要进行人工血小板计数。［关键词］血小板计数;Sysmex-lSOOi血液分析仪;红细胞平均体积［中图分类号］R44［文献标识码］B［文章编号］1673-7210(2010)05(c)-072-03InfluencefactorontherulesofduplicateexaminationofSysmex-lSOOibloodanalyzerforplateletcountZHANGXia(ClinicalLaboratoryoftheFirstPeople'sHospitalofKunming,Kunming650011,China)［Abstract］Objective:TodiscussandestablishproperduplicateexaminationrulesofSysmex-18001bloodanalyzerforplateletcount.Methods:WeusedSysmex-lSOOibloodanalyzertodetect450bloodsampleswhichweredividedintothreegroups,groupofredbloodcellwithdifferentMCV,groupofplateletwithdifferentinternalfactorandvariedmethodsoftakingbloodgroup.Countedtheplateletbymanudlunderthemicroscopeandreviewedbybloodsmear・Results:@MCV>80fland70fKMCV<80fl,therewasnodiffereneebetweeninstrumcntandhandforplateletcount(P>0.05),butwhen60flWMCV〈70fl,thedifferenceexisted(P<0.05).MCV<60fl,also,therewasdifferencebetweenthetwomethods(P〈0.05)•②Differeneewasobservedbetweenthemethodsofinstrumentandhandwhenbigplateletorabnormalplatelethistogram(P<0.05).Whenplateletaggregationtherewassignificantlydifferentbetweenplateletcountmeasuredbyequipmentandbloodsmear•③IftheresuItsofbloodanalysiscouldbeacceptedunderdifferentmethodsoftakingblood,ithasadvantagesofstableresults,plentyofsamplesandeasytoreviewbyusingtheEDTA-k2vacuumbloodcollectiontube・Venousblood,whilegivingprioritytotheclinicalsituationcanselectthesampletypeandbloodcontainers.Whenthrombopeniawasdetected,thevenousbloodshouldbetakenagainandcounttheplateletbyhands.Conelusion:MCVW70floftheredbloodcell,thesampleswasnotqualified,theresultsandtheclinicalfindingsdonotmatch,itisnecessarytocounttheplateletbyhands.[Keywords]Plateletcount;Sysmex-1800iBloodAnalyzer;Meancellvolume自动血细胞分析仪首次使用至今已有半个世纪，引入我国也近20年。如何能够让全白动血细胞分析仪更好地为临床服务，一肓是备国血液学专家共同关注的问题［1］。据文件检索,I询未见对各项影响设备血小板计数因素而制订的复检原则报道。因而针对影响Sysmex-lSOOi血液分析仪血小板计数的因索，探讨并制订相关复检原则有重要的实用价值。1材料与方法1.1材料1.1.1仪器Sysmex-lSOOi血液分析仪由希森美康医用有限公司生产。Olympus双目显微镜由I」木Olympus公司生产。改良牛巴血细胞计数盘由上海市求精生化试剂仪器公司生产,EDTA-K2由江苏吉春医用器材有限公司提供。1.1.2试剂血液分析仪试剂由希森美康医用有限公司提供的原装配套试剂。血小板手T法稀释液(草酸钱稀释液)严格按《全国临床检验操作规程》［2］配制。1.1.3标本来源450例标本...
