多层螺旋CT在成人肠套叠诊断中的应用价值及病因分析［摘要］目的探讨多层螺旋CT在成人肠套叠诊断中的应用价值，着重研究其对成人肠套叠及其原发病诊断的准确性。方法该研究整群选取并冋顾性分析该院2008年4月一2015年4月收治并经手术证实的52例成人肠套叠患者；手术方法为传统开腹手术,术式为:单纯病变切除术，连同病变在内的部分肠切除术，包扌舌相应的系膜及区域淋巴结在内的肿瘤根治术。术前以多层螺旋CT轴位薄层扫描(平扫或平扫+增强)结合多平面重组技术，分析肠套叠CT征象，查找其原发病灶。结果手术结果显示，有49例属于顺行性肠套叠，3例属于逆行性肠套叠，其中冋肠-结肠型肠套叠26例，结肠-结肠型肠套叠11例，小肠-小肠型肠套叠12例，结肠-直肠型肠套叠3例。术后病理诊断结果显示，良性病变23例，恶性病变28例。术前CT诊断肠套叠47例，准确率为90.38%,正确诊断出原发病变38例，准确率为73.08%。结论多层螺旋CT对于成人肠套叠诊断具有重要价值，能够发现其原发病变，对于其后期诊治起着指导意义。［关键词］多层螺旋CT；成人肠套叠；应用价值；病因［中图分类号］R574.3［文献标识码］A［文章编号］1674-0742(2016)01(a)-0186-03ApplicationValueandEtiologicalAnalysisofMultisliceCTinDiagnosisofAduItIntussusceptionLONGLin-jun,HONGDong-yuCT/MRIroom,HepuPeople，sHospital,Beihai,GuangxiProvince,536100China[Abstract]ObjectiveTodiscusstheapplicationvalueofmultisliceCTindiagnosisofadultintussusceptionandparticularlyresearchitsaccuracyindiagnosisofadultintussusceptionandprimarydisease.Methods52casesofpatientswithadultintussusceptionconfirmedbyoperationinourhospitalfromApril2008toApri12015wereretrospectivelyanalyzed,theoperationmethodwastraditionalopensurgeryandtheoperativeprocedureswere:purelesionsresection,partialenterectomytogetherwithlesions,includingtumorradicaloperationcoveringcorrespondingmesenteryandregionallymphnodes,beforeoperation,thepatientsweregivenmultisliceCTaxialthinlayerscanning(plainorplainandenhanced)combinedwithmulti-planarreconstruetion,CTcharacteristicsofintussusceptionwereanalyzedandtheirprimarylesionsweresearched.ResultsTheoperationresultsshowedthat49casesbelongedtoantegradeintussusception,3casesbelongedtoretrogradeintussusception，26casesbelongedtoileo-colicintussusception,11casesbelongedtocolon-colonintussusception,12casesbelongedtosmallbowel1-smallbowellintussusceptionand3casesbelongedtocolon-rectumintussusception,thepostoperativepathologicaldiagnosisresultsshowedthat23caseswerebenignlesionsand28casesweremalignantlesions,beforeoperation,therewere47casesdiagnosedwithintussusceptionbyCT,且ndtheaccuracyratewas90.38%,therewere38casescorrectlydiagnosedwithprimarylesionsbyCT,andtheaccuracyratewas73.08%.ConclusionMultisliceCTisofimportantvaluetothediagnosisofaduItintussusception,whichcanfinditsprimarylesionsandplayaguidingroleinitsdiagnosisandtreatmentinlatestage.[Keywords]MultisiiceCT；AduItintussusception；Applicationvalue；Etiology肠套叠是指肠管的一部分及其相应肠系膜结构套入与其相连的肠腔内。成人肠套叠少见，约占肠梗阻的1%,肠套叠的5%。随着近年来多层螺旋CT的广泛应用，其对成人肠套叠诊断的重要价值日益得到重视。为了评价多层螺旋CT在成人肠套叠诊断中的应用价值，该研究整群选取回顾性分析该院2008年4月一2015年4月收治并经手术和病理证实的52例成人肠套叠患者，在分析其病因的基础上，以多层螺旋CT轴位薄层扫描（平扫或平扫+增强）结合多平面重组技术，着重研究其对成人肠套叠及其原发病诊断的准确性，从而提升其救治效果以及预后质量，现报道如下。1资料与方法1.1一般资料整群收集该院2008年4月一2015年4月间经手术和病理确诊的52例成人肠套叠，其中男性37例，女性15例；年龄19〜88...
